What we collect through this website, why, and the choices you have. Written to be read, not skimmed past.
What we collect
We only collect personal information you choose to give us through this website:
- Enquiries (contact and Find Support forms) — your name, contact details, the general kind of support you are asking about, suburb or postcode, and anything you write in the message box.
- Referrals — the participant and referrer details on the referral form, submitted with the participant’s consent.
Our enquiry forms deliberately do not ask for diagnoses, health records or NDIS numbers. Those conversations happen with a person, after you have chosen to work with us.
How we use it
- To respond to your enquiry or referral.
- To set up and deliver the supports you ask for.
- To meet our obligations as a registered NDIS provider.
We do not sell personal information, and we do not use enquiry details for advertising.
Website analytics
We use website analytics (via Google Tag Manager and a website analytics tracker) to understand how the site is used — pages visited, buttons clicked, forms started and completed. Analytics events never include your name, contact details, postcode or anything you type into a form. The tracker respects your browser’s Do Not Track setting.
Address autocompletion
The referral form suggests addresses as you type using Google’s Places service. Those requests are made from our server, and only the text you type into the address field is sent for matching.
Storage and security
Form submissions are transmitted over HTTPS to our website platform and handled by the Famycare team. Access is limited to staff who need it to respond to you.
Access, correction and deletion
You can ask us what personal information we hold about you, ask us to correct it, or ask us to delete information we are not required to retain. Contact info@famycare.com.au or call 03 8752 2335.

If you have a concern about how we have handled your information, see feedback and complaints. You can also contact the Office of the Australian Information Commissioner at oaic.gov.au.
Note for the Famycare team: this is a plain-language draft grounded in what the website technically does. It is not legal advice. Have it reviewed against the Australian Privacy Principles and your NDIS record-keeping obligations, add your retention periods, and delete this note once that review is complete.
